About the Job
We are seeking an experienced Registered Nurse (RN) to provide ongoing community nursing support to an NDIS participant in their home in Plumpton, NSW.

This is an opportunity for a confident and clinically experienced RN to provide consistent, person-centred nursing care and work collaboratively with the participant, support team and treating health professionals.

Key Responsibilities:
  • Permanent male IDC catheter management, including scheduled catheter removal, reinsertion and changes.
  • Ongoing catheter assessment, troubleshooting and monitoring.
  • Clinical documentation and nursing progress notes.
  • Diabetes management, including developing and reviewing a Diabetes Management Plan in consultation with the participant’s treating team and current medical orders.
  • BGL monitoring and insulin management guidance.
  • Establishing and following clear hypoglycaemia and hyperglycaemia management and escalation protocols.
  • Skin integrity assessment and wound-related nursing support as required.
  • Ongoing clinical assessment and care planning.
  • Communicating effectively with the participant’s treating health professionals and support team.
  • Identifying and escalating changes in the participant’s health or clinical condition.

About You: We are looking for an RN who is:
  • Registered with AHPRA.
  • Experienced in community nursing or complex disability care.
  • Confident and experienced in male IDC catheter management, including catheter changes.
  • Experienced in diabetes management, BGL monitoring and insulin support.
  • Confident with clinical assessment, documentation and care planning.
  • Able to work independently in a participant’s home.
  • Reliable, professional and committed to providing high-quality, person-centred care.
  • Comfortable communicating and collaborating with a multidisciplinary treating team.

Requirements:
  • Current AHPRA Registered Nurse registration.
  • Relevant nursing experience, particularly in community, disability or complex care.
  • Current First Aid & CPR.
  • NDIS Worker Screening Check.
  • Comprehensive car insurance and reliable vehicle.
  • Current Australian driver's licence.
